Bloodletter, an exceptional unique item, gives more than decent damage for a one-handed sword, with a very high enhanced damage plus an added base damage. Combined with an increased Attack Rating and Attack Speed, Bloodletter can do major damage.
However, due to the nature of its skill bonuses, Bloodletter is generally meant for Barbarian use. Since many high-level Barbarians often choose to put levels into Whirlwind, Bloodletter is a good weapon for those Barbarians, as it also gives a large boost to Sword Mastery.
On the other hand, Bloodletter is a Gladius - some Barbarians may dislike using Whirlwind without the extended radius of a long weapon such as a Spear or Polearms. The bonus may make up for it nonetheless.
Bloodletter also has several other bonuses. Although its slower Stamina drain is most likely pointless, since most heroes would generally put many points into Vitality, its life leech is useful. When using Whirlwind, Barbarians put themselves in close proximity to enemies. In Nightmare and Hell difficulties, when enemies do much more damage, life leech is often necessary to survive a mob.
