The Forgotten Ruins is a Ruins dungeon found in the Dahlgur Oasis in Act II of Diablo III.
This is where the Horadrim managed to hide Zoltun Kulle's severed head, leaving the Ancient Guardian to protect it.
There is an achievement for finding the head in less than 90 seconds after entering the dungeon.
