Co-spawn with Matriarch's Bones[]
I noticed Jar of Souls and The Matriarch's Bones seem to be a mutually exclusive guaranteed co-spawn - in other words, every game will have one or the other, and never both. They both appear in the fake Defiled Crypt. I noted 9 games in which I fully explored all 3 Defiled Crypts: All fit this theory, 4 had the Matriarch's Bones, and 5 had the Jar of Souls (1 in Campaign Mode), none had both.
