The Guardian (quest)

The Guardian (quest) is the final quest of Act III. It's initiated by Ormus when players near Travincal or smash the Compelling Orb. The quest involves killing Mephisto, the Lord of Hatred and is completed after players manage to do so.

See the Mephisto article for tips on how to defeat the Lord of Hatred and complete the quest. You cannot enter the Durance of Hate until you smash the Compelling Orb with Khalim's Will (Item).
